What to Bring to Your Appointment

Bringing the right items to your appointment saves time, prevents delays, and helps your doctor give you the best care. Use this checklist so you are never caught without something you need.

Insurance Card and Photo ID

Bring your current insurance card and a government-issued photo ID. The front desk needs both to verify coverage and check you in.

If your insurance recently changed, bring the newest card to avoid billing problems.

A List of Your Medications

Bring an up-to-date list of all prescriptions, over-the-counter drugs, vitamins, and supplements, including doses — or bring the bottles themselves.

Include any known allergies or past reactions.
